Fictional composite example · Not a real client case

Sample What Changed update

Each month of Monthly Follow-Through closes with one page like this: four short states, written to be read in a minute. This is version 1.1 of the same fictional case as the Sample Clarity Brief, covering what arrived after that Brief's record window closed. Everything named below is dated after the Brief's own sample record cutoff of 7 July 2026, so nothing here repeats what the Brief already held.

This update is a fictional composite, not a real client document.

Sam is not a real person. Every date, letter and detail below was invented for this example, and no real client appears in it. The sources behind the case are real and are listed, with what each supports, in the full sample Brief this page continues.

The month, in four states

What changed since the Brief

  • New

Two records arrived after the Brief. The 1 August 2026 NHS repeat panel reports TSH and free T4 within that laboratory's ranges again, with ferritin still near the low end. The 7 August 2026 clinician letter makes no diagnosis and proposes a review in six months unless something changes sooner.

  • Changed

The supplement was paused before the August panel, so that result is the first one taken without it. Sleep is marginally better since fixing a consistent wake time; daytime energy is unchanged.

  • Still unclear

Why the April result differed. Two later panels within range narrow it, but they cannot separate a true transient rise from an assay difference or from supplement timing.

  • Ask next

Is six months the right interval given the ferritin has not moved, and which specific symptom change should trigger an earlier review instead of waiting?

The full picture behind these four states, including the timeline, the source list with publication and review dates, and the questions for the next appointment, is in the Sample Clarity Brief.
